Prior to your child’s start date, the following forms must be completed and returned to Giggle and Grow Childcare & Preschool:

Please keep us informed of any changes in medical or emergency information. In order to maintain compliance with the Arizona Department of Health Services and Childcare Licensure, we must maintain current and accurate records at all times.

Submit Your Enrollment Application

Download the Fillable Child Enrollment Form above and fill it out. When completed, you may drop it off in-person or you can submit it by drag and dropping it below. Then someone from our facility will contact you with next steps. Thank you for your interest in enrolling your child in our daycare and preschool!
